Risk form

I acknowledge, on my personal behalf and as a Guardian, that it is essential that my/the Minor's participation in the activity be governed by the terms and conditions below. Having reviewed the activities offered by Lokal Adventure, its representatives, employees, directors, members, subcontractors and/or any other affiliated person or entity, I hereby agree and confirm that:

1. I declare that I have been informed of the independent relationship between Casela Limited (BRN No. C13119467) and Lokal Adventure Ltd, in connection with the activity in which I/the minor participate. I further understand and agree that Lokal Adventure Limited will not be held liable, in any way whatsoever, for any incident that may occur during the activity;
2. I understand that the selected activity involves risks that cannot be eliminated without destroying the very element that makes that activity so unique.
3. I also understand that Lokal Adventure considers it important to warn me/the Minor in advance of all risks associated with the selected activity;
4. I acknowledge that I have been informed of the difficulty level of the selected activity;
5. I am fully aware and agree to assume full responsibility for the risks associated with the activities that I/the Minor have voluntarily decided to participate in (The Minor must remain under the responsibility of the Guardian for the entire duration of the selected activity);
6. I am not/the Minor is not under the influence of drugs or alcohol;
7. I am/the Minor is physically and mentally capable of participating safely and, to my knowledge, I/the Minor do not suffer from any health problems and/or disabilities whatsoever, likely to endanger my life/the Minor's life;
8. I/the Minor have been informed that it is strictly forbidden to participate in these types of activities during pregnancy;
9. I understand that I will be solely responsible for non-compliance with the safety measures and instructions relating to the activity, by me/the Minor, and that non-compliance with these authorizes Lokal Adventure to immediately terminate the activity;
10. I declare that I am/the Minor is able to handle and use the equipment provided by Lokal Adventure (the “Equipment”) and I agree that the use of the Equipment is at my/the Minor's own risk;
11. I/the Minor must, at all times, wear a safety helmet and/or any other Equipment that has been provided to me/provided to the Minor, for my/the Minor's participation in the activity;
12. I agree that, during the duration of the activity, the Equipment will be under my custody/the Minor's custody, until it is returned to Lokal Adventure. I agree to assume full responsibility for all costs incurred as a result of any damage caused by me/the Minor to the Equipment as a direct result of its misuse or non-compliance with the instructions provided by Lokal Adventure, its representatives, employees and/or any other affiliated person or entity of Lokal Adventure;
13. I agree to release Lokal Adventure, its employees, agents and/or any medical personnel from any claim, request for compensation or prosecution, whether made by myself, or by the Minor, or by my successors/the Minor's successors (“successors” includes legal guardian, administrator, parents, heirs and estate), for any physical, moral or material damage that I/the Minor may suffer in connection with the activity, unless it results from gross negligence of, or negligence on the part of, Lokal Adventure, its employees and/or agents;
14. I undertake, on my personal behalf and as Guardian of the minor, to indemnify and hold Lokal Adventure, its employees or agents harmless from and against all claims and actions whatsoever that may at any time be suffered or incurred by, or against any of them, as a result of, or in connection with, my acts and actions/the Minor's acts and actions and my participation/the Minor's participation in the activity;
15. I declare that I have verified that my/the Minor's vacation insurance covers the selected activity;
